What Happened
A case management inspection found a broken infant highchair with missing hardware, creating an immediate safety risk and resulting in a Type A citation. The visit also confirmed a personal-rights issue involving staff yelling at children inappropriately.
Observations
- The infant room contained a wooden highchair missing four screws that help secure the child
- The director reported the child was not buckled in when the tray and center section fell out
- The child fell out of the broken highchair
- Multiple witnesses confirmed staff #1 yelled at children inappropriately for disciplinary purposes
