What the inspector wrote

On 7/21/22, Licensing Program Analyst (LPA), Tyra Block made an unannounced initial 10-day visit to investigate the above referenced allegation. LPA met with Licensee, Janet Andrade. Also present were 2 staff and 9 chidlren. All staff has criminal record clearances. LPA toured the facility and obtained a copy of the children’s roster. LPA conducted interviews with staff. Based on the information obtained during interviews, documents reviewed, and observation it is determined that the allegation is SUBSTANTIATED. The deficiency is being cited on the attached LIC 9099D. The Notice of Site Visit was provided, and LPA observed posting. Licensee is advised it must remain posted for 30 days. LPA Block informed the licensee to provide a copy of this licensing report dated 7/21/22 that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. LIC 9224 was provided along with Parent Notification Requirements. An exit interview was conducted, with Janet Andrade. This is an amended report to revise citation to Type A.

    101223(a)(2)-Personal Rights: The licensee shall ensure that each child is accorded the following personal rights:To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs.This requirement was not met as evidenced by: Based on observation and interviews the playground equipment such as moneky bars and canopies are not anchored appropriately. This poses an immediate health and safety risk to chidlren in care.

  • FIXTURES, FURNITURE, EQUIPMENT AND SUPPLIES

    101239(e)(4)-Fixtures, Furniture, Equipment and Supplies: All toilets, handwashing and bathing facilities shall be maintained in safe and sanitary operating condition...This requirement was not met as evidenced by: Based on observation and interviews the toilets are not maintained in working order and has been an ongoing issue at the center. This poses an immediate heatlh and safety risk to children in care.
