What the inspector wrote

On 3/24/2022 @ 2:30PM, Licensing Program Analysts (LPAs) Patrick Ma and Keturah Lane conducted an unannounced case management inspection. LPAs toured the classrooms upon entrance. There were 8 children at the facility with 5 teachers today. Appropriate ratios were observed. On 2/14/22, Director self-reported a possible personal rights violation against child #1. LPA’s interviewed staff #1, staff #2, staff #3, and site director, Teresa West. No deficiency was cited, however, further investigation is needed. Exit interview was conducted with Teresa West. LPAs provided a copy of this report and appeal rights were discussed and provided today. Notice of site visit was observed posted. This notice shall remain posted for 30 days.
