What the inspector wrote

Based on interviews conducted, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. Deficiencies are cited per California Code of Regulations, Title 22, Division 6 on the attached 9099D. An exit interview was conducted and report was reviewed with Director, Felicia Early. A notice of site visit was given, and must remain posted for 30 days.

Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.

Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    Responsibility for Providing Care & Supervision - ..."No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation." This requirement is not met as evidenced by: Based on interviews conducted, C1 was left without the visual observation of a teacher. C1 climbed up a play structure, was unable to stretch to reach a platform and fell 5 feet to the ground sustaining injury. This poses a potential health and safety risk to children in care.
