Licensing Program Analyst (LPA) Jennifer Lott conducted a Case Management visit to deliver an amended plan of correction. LPA was greeted at the front door by Director, Felicia Early and granted entry after identifying herself and disclosing the purpose of her visit. The purpose of LPA's visit was to deliver an amended plan of correction for a visit that was conducted on 07/25/2022. During today's visit, LPA Lott met with Director, Felicia Early and obtained the Director's signature on the amended report LIC 809-D dated 07/25/2022. An exit interview was conducted and a copy of this report was provided. A notice of site visit was given and must remain posted for 30 days. LPA Lott informed Director Early that this report dated 07/25/2022, document(s) one Type A citation. Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Lott informed Director Early, to provide a copy of this licensing report dated 07/25/2022 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
