What the inspector wrote

On 04/10/23 at 10:00am, Licensing Program Analyst (LPA), Saraliz Velando and Licensing Program Manager (LPM), Monica Cuddy conducted an Office Meeting in the San Diego Child Care Regional Office to discuss the facility's recent citations issued. Meeting was conducted with the Everbrook District Manager, Tammi Reliford and Everbrook Director, Patricia Munoz. 2/15/23 – Personal Rights -101223(a)(1) - Staff did not attend to a child that was crying at the playground as she had urinated on her clothes, shoes and was soaked in urine. SUBSTANTIATED (Type B), Personal Rights -101223(a)(3) - Staff did not assist child with toileting as needed. Child was not accorded dignity. SUBSTANTIATED (Type B) 10/13/22 - Teacher Qualifications and Duties 101216.1(b)(1) - Staff is not a fully qualified teacher due to missing units. (Type B) 9/29/22 - Responsibility for Providing Care & Supervision 101229(a)(1) - Facility failed to provide visual supervision resulting to a child leaving the classroom. (Type B) 4/14/22 - Staff violated the personal rights of children in care on more than one occasion by handling children in a rough manner and not changing diapers in a timely manner. (Type A) 4/14/22 - Personal Rights 101223(a)(2) – Staff failed to change the diapers of children in care in a timely manner. (Type B) (See 809-C) 4/14/22 – Personal Rights 101223(a)(3) Children are not being afforded their personal rights. SUBSTANTIATED (Type A), 101223(a)(2) Staff are not changing diapers in a timely manner. SUBSTANTIATED (Type B) LPM Cuddy discussed with Facility Representatives the above deficiencies. Facility representatives were provided the following resources: A link to the Care Tool was provided https://cdss.ca.gov/inforesources/cdss-programs/community-care-licensing/inspection-process-project/care-tools , a Technical Support Program (TSP) brochure, for questions related to the TSP, email: childcaretechnicalsupport@dss.ca.gov, active supervision handout, ccld.childcarevideos.org, provided a copy of the children’s personal rights and the most recent Quarterly Update. Advocate information was provided: (714)-703-2800 or childcareadvocatesprogram@dss.ca.gov, sdincidentreports@dss.ca.gov and the Duty Line (619)-767-2248. Licensee agreed to be referred to TSP. Licensee was reminded that LPA requested the following items to complete the Director packet for Patricia Munoz: a corrected LIC503, transcript from University of Riverside, LIC200A, and clear copy of transcript from Mt. San Jacinto College. Licensee was informed that the department will be conducting follow up inspections in the coming months to ensure compliance with regulations is maintained. Facility Representatives were informed that repeated violations or failure to comply with licensing laws and regulations may result in administrative action taken by the Department. Facility Representatives, Tammi Reliford and Patricia Munoz stated they understood and will ensure the facility complies with all regulations and laws governing Child Care Centers.
