What the inspector wrote

On 10/13/22 at 9:09AM, an unannounced case management inspection was made by Licensing Program Analysts (LPAs) Saraliz Velando and Nancy Diaz to follow up on an incident report that occurred on 9/30/22 at approximately 4:25PM. Upon arrival, LPAs met with Interim Director, Sherri Brown. A tour of the facility was conducted and LPAs observed 13 children in care. The facility self-reported the incident to the department by email on 10/10/22. During the incident of 9/30/22, a child was found outside his classroom door by a staff member and brought back to his classroom. According to staff, the child was missing for approximately 37 seconds. Staff member involved was interviewed and based on the information, it appears that care and supervision was not provided resulting in a one-year old infant crawling outside of the classroom. Type B Violation was cited. Civil penalty is being assessed at this time for Repeat Violation. Refer to the next page LIC 809D for deficiency citation. Facility was provided a copy of the appeal rights. Exit interview was conducted and report was reviewed with Substitute Director, Jennifer Sweet. A notice of site visit was given and observed posted, this notice shall remain posted for 30 days.

Ms.Saldarriaga is currently not enrolled in any ECE class.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION FOR INFANTS

    Responsibility for Providing Care and Supervision for InfantsIn addition to Section 101229, the following shall apply: (1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement was not met as evidenced by: Based on Staff interview, on 9/30/22 a one year old child crawled out of his classroom and was unsupervised for approximately 37 seconds.This poses a potential health and safety risk to children in care.
