Follow-up on corrections·December 14, 2022

On 12/14/22, Licensing Program Analysts (LPAs), Saraliz Velando and Selina Siao, conducted a Case Management inspection. Acting Director, Sherri Brown was present at the facility. LPAs toured the facility and there were 17 children total and 4 staff. The following ratio was observed in the Infant room 9 infants supervised with the two staff present, Sherri Brown and Julissa Corona. The following ratio was observed playing outside as part of the Toddler Room 8 toddlers with staff members Michelle Dossett and Elizabeth Angeles. Type B deficiency was cited today. Refer to the next page LIC 809-D for type B deficiency citation. An exit interview was conducted with Sherri Brown and appeal rights were provided. A notice of site visit was provided, and it must be posted at the facility for 30 days. Failure to keep notice posted will result in a civil penalty of $100.00.
