On 10/7/25 at 10:39 a.m. Licensing Program Analyst LPA Renita Rodriguez made an unannounced case management inspection for a self reported incident that occurred on 9/23/25. LPA met with Director Audrey Hutchinson. Ratios observed today 23 children and 3 staff. The incident occurred on 9/23/25 at 10:00 a.m. during outdoor activities on the play ground. The ratio during the time of the incident was 14 children and 2 staff. A water bottle belonging to a staff member S1 was placed on a non- operational decorative wooden water tower structure. The water bottle fell off the structure immediately after being placed on the structure. Staff S1 looked down at the water bottle and also at the child in the vicinity of where the bottle fell. S1 asked C1, "Are you okay?". S1 was looking the child over and saw blood on their hands. S1 obtained wipes to clean off the child's hands. S1 was trying to figure where the blood was coming from and noticed that it was from C1 head. Director was called on walkie talkie to provide assistance to the incident. Parent was called and C1 was picked up. C1 was taken to the hospital and staples were placed on the area. C1 returned to care at the center on 9/24/25 with verbal instruction from parent stating the area needed to remain from getting dirt. No deficiencies cited today. Exit interview conducted and notice of sit visit was provided to Director Audrey Hutchinson.
