On 10/17/25, at 8:27 a.m. Licensing Program Analyst (LPA) Renita Rodriguez conducted an unannounced case management inspection to deliver an amended report. A tour of the facility was conducted with the Director, Stassia House. There were 7 children and 1 staff present today. LPA observed the outdoor playground area and rubber flooring. The flooring on the play ground area appears to absorb falls. LPA stepped down on the flooring to check for buoyancy. The Director states a company has been sent pictures of the playground area. The company will be checking the playground area on 10/20/25. The Director will provide LPA with the information by 10/31/25 for the company's quote and how they will proceed with the repairs and or replacement of the rubber flooring. LPA obtained the proof of communication with the center and the company that will be providing a quote for the playground flooring. LPA obtained the photo of the mixture that was being used to patch the holes in the flooring. Exit interview conducted and report was reviewed with the Director Stassia House. A notice of site visit was given and must remain posted for 30 days.
