What the inspector wrote

On June 2, 2023 at 10:10 a.m. Licensing Program Analyst (LPA) Leilani Curtis visited the facility to conduct an annual inspection. Upon arrival LPA met with Licensee Joanne Paz and proceeded to tour the facility. LPA provided the LIC 125, Entrance Checklist to Ms. Paz. Also present were 32 children with 3 staff members. Appropriate ratios and capacity were observed. Staff members have the required background clearances and are associated to the facility. No excluded individuals are present. LPA observed that licensee is using one room that is not specified on the license. The facility is currently licensed to use rooms 1, 6, 12 & 17 for preschool children. LPA observed that care and supervision is being provided in room #\342\200\231s 6, 16 and 17. Room# 16 is not a licensed room under the preschool license. Room #16 is currently licensed for infant use under facility number 376105089. The licensee states that room #1 is used as an infant room. LPA advised Licensee that an updated application for both the preschool and infant licenses will need to be submitted to the department to request a room change. Licensee will also notify LPA if she decides to return room #16 back to an infant room and room#1 to a preschool room so they may be reinspected. Furniture and age-appropriate equipment is in good condition indoors and outdoors. Children's toilets and hand washing facilities are sanitary. Rooms are safe and clean. Food preparation area is clean, food and beverages are stored in covered containers at 45 degrees F or less if required, and storage containers for solid waste are covered. Drinking water is readily accessible inside and outside the classroom. All disinfectants, cleaning solutions, and other hazardous items are inaccessible to children through latches and locks. Storage area for poisons is locked. Medications are kept in a safe place inaccessible to children. The outdoor play area is fenced. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The areas around or under high climbing equipment, swings, slides, and similar equipment is cushioned with material that absorbs a fall. The outdoor play area has canopies used for shade. There are no bodies of water or weapons at this facility. Fire drills are being conducted every 6 months. There is an operational carbon monoxide detector at the facility. First Aid/CPR certifications were reviewed and are in compliance. This facility provides Incidental Medical Services \342\200\223 IMS. The licensee states that one child occasionally brings an inhaler with him to the facility. The medication was not at the facility during the inspection. The facility does not have an IMS Plan of Operation on file. LPA advised Licensee to submit a written Plan of Operation to provide IMS to LPA via email by 6/30/23. Admission agreement, emergency information and medical assessment forms were reviewed for some children. Sign in/sign out sheets are well maintained. Staff records contain documentation of education, training, and/or experience. Two staff members do not have verification of a tuberculosis clearance obtained within one year of hire or licensure. Menus are posted. The licensee states that the facility water has not been tested for lead. LPA reviewed the following with licensee: Written Directives for Lead Testing in Water, PIN 21-21.1 CCP dated 12/28/22. LPA obtained an updated Emergency Disaster Plan for Child Care Centers- LIC610, a Personnel Report-LIC500, Children\342\200\231s Roster-LIC9040 and Parent Handbook. Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Licensee Joanne Paz.
