Inspector’s narrative

What the inspector wrote

On 09/13/2022 at 10:31am, Licensing Program Analysts (LPA), Samantha Clenista and Leilani Curtis conducted an unannounced Post Licensing inspection. Upon arrival, LPA\342\200\231s met with Owner, Joanne Paz. LPA's conducted a tour of the facility and observed 22 children with 6 staff. On 04/27/22, LPA issued a provisional license (expiring 10/01/2022) due to their fire clearance being provisional and facility was still in the process of establishing a permanent outdoor playground. LPA received a regular fire clearance on 09/09/2022. Purpose of this visit is to follow-up and inspect facility\342\200\231s permanent outdoor playground that they had been working on since facility was originally licensed on 04/27/22. Facility is currently operating under the capacity of the provisional license issued on 04/27/22, which listed in error a capacity of 32 children instead of 20. A new license reflecting the capacity of 20 children has been updated and issued. During the inspection, Ms. Paz expressed that she would like to apply for an increase capacity due to adding the outdoor playground in addition to the already licensed indoor playground. Facility is currently licensed to care for up to 20 infants (6 weeks-24 months) in the following rooms: 2, 3, 11 and 16. Ms. Paz stated she understands that the facility cannot exceed the licensed capacity of 20 infants. Ms. Paz stated that she will ensure that the capacity does not exceed by temporarily reducing the number of children attending. Facility has an approved playground waiver on file to allow the facility to use Room 3 as an indoor playground. This indoor playground was measured by LPA Clenista on 04/12/22 and it measured at 901.4 sq. ft, sufficient for 12 children at one time. Facility has added an outdoor playground for the infants since then. LPA\342\200\231s inspected and measured the infant outdoor playground during today\342\200\231s inspection. Playground is located in the parking lot, left of the building (facing it). Area was observed fully fenced, which was higher than 4ft. tall. Ms. Paz stated she plans to install turf. LPA's asked Ms. Paz to submit pictures after the installation to LPA Clenista. The infant and preschool play area is separated by a 2 ft. fence. LPA\342\200\231s observed the infant area to have a canopy used for shade and had age appropriate play equipment. Area was observed to be free from hazards and in good repair. Infant side of the playground measured at approximately 361.76 sq. ft., sufficient for only 4 children at one time. Facility is requesting that the indoor playground be counted in addition to the outdoor playground. Areas of both combined equate to 1,263.16 sq. ft., sufficient for 16 children at one time. Due to the facility requesting a license for 32 infants, the following is needed: Change of Capacity application (LIC200A) Updated facility sketch for both indoor and outdoor activity space $25 processing fee (check or money order) Waiver request to share the outdoor and indoor playground on a scheduled basis Playground schedule showing that no more than 4 infants will utilize the outdoor playground at one time and no more than 12 infants will utilize the indoor playground at one time Before the department can issue a regular license, a playground waiver must be submitted and approved. A license for 32 infants in Rooms 2, 3, 11 and 16, maybe be granted upon receiving the above corrections, playground waiver approval and final file review. Exit interview was conducted with Ms. Paz and Ms. Hewitt. Notice of Site Visit was provided and is to remain posted for 30 days.
