What the inspector wrote

THIS IS AN AMENDED REPORT DELIVERED 1/13/26. On 11/24/25 at 8:50 AM, Licensing Program Analyst (LPA) Renita Rodriguez conducted an unannounced case management visit to follow up on a self reported unusual incident that happened on 10/2/25. Upon arrival, LPA was greeted by Deanna Muenkel and toured the facility. LPA observed 34 children and 8 staff. LPA toured the playground area where the incident occurred. On 10/2/25, staff observed inappropriate behavior between two children. The behavior was addressed with both the children and their parents. A staff meeting was subsequently held, during which the team reviewed safety and supervision practices for outdoor activities. The table previously used to place children\342\200\231s water bottles, located near the bathroom, will no longer be used by children during outdoor activities unless a staff member is providing direct supervision. At the time of the incident, there were no visual obstructions preventing staff from supervising the table; staff were not standing directly next to it but had a clear line of sight. To enhance supervision moving forward, children\342\200\231s water bottles are now placed on the picnic tables located closer to the play structure. This adjustment was made to ensure improved visibility and oversight during outdoor play. There were no deficiencies cited at this time. A notice of site visit was given and must remain posted for 30 days. The report was reviewed with Deanna Muenkel.
