Pre-licensing visit·October 19, 2022

On 10/19/2022 at 9:50am, Licensing Program Analyst (LPA), Samantha Clenista conducted an announced Pre-Licensing inspection. Upon arrival, LPA met with Facility Representative, Alexandra Ellam. LPA took a tour of the facility per facility sketch. This facility houses a preschool, toddler and infant program. The Preschool Program (children ages 2-5) will operate in Rooms 103 and 201. The Toddler Option Program (18-36 months) will operate in Room 202. Fire clearance was granted on 10/11/2022. If medications are on site, Ms. Ellam stated that will be stored in the director's office in a locked container. All indoor and outdoor activity space utilized for the children were inspected and measured today. The following measurements were taken: Room 103 (Preschool Room)= 517.96 sq. ft. Room 201(Preschool Room)= 425.82 sq. ft. Room 202 (Toddler Room)= 404.82 sq. ft. LPA observed all indoor play space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. There is a total of 4 toilets, 1 urinal and 6 sinks available for the children. Children are to be escorted and supervised by staff to and from the bathroom and under direct visual observation. There is one separate bathroom that consist of 1 toilet and 1 sink is used by staff and is also designated for use by children who are ill. The director's office is the isolation area for children who are ill. There is a carbon monoxide detector on site located room 103 and 201. The kitchen area currently includes a sink, two ovens, a stove, a microwave and one refrigerator. Area was was observed free of rodents and/or vermin. Sign-in sheet will be electronic using the application called Bright Wheel. Ms. Ellam stated there are no firearms/weapons on site. LPA observed a water fountain located on the premises and it not appropriately inaccessible to children. The outdoor playground area was inspected and measured. Area was measured at approximately 3,435 sq. ft., which will accommodate no more than 45 children at one time. When LPA inspected the outdoor playground, LPA observed a big canopy used for shade. Play area has only one playhous e and no others toys or play equipment. Area under the shade has enough wood chips. Ms. Ellam stated she has a big play structure that is scheduled to come in next week. Area is fully fenced (above 4ft. high). Ms. Ellam stated that drinking water will be readily accessible inside and outside the classroom via water bottles/cups and refillable water stations, however, LPA did not observe any on site. Ms. Ellam is reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Incidental Medical services (IMS) policy was discussed. For IMS information see Evaluator Manual \342\200\223 Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA and applicant discussed California Megan's Law and LPA provided: www.meganslaw.ca.gov . Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPA reviewed the updated LIC311A with the facility representative today and reminded to keep anything that reads, \342\200\234Keep Out of Reach of Children\342\200\235 inaccessible to children. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Prior to licensure, the following must be submitted/completed: Label all bathrooms do designate which bathrooms are used for children versus adults Obtain a first kid Submit and obtain an approved playground waiver to allow the toddlers to share the infant playground Purchase/install more play equipment for the infant/toddler and preschool playground Post all required documents for the parents and employees Ms. Ellam stated she will purchase/install refillable water stations as a form of accessible water to children inside and outside the classroom Make the water fountain located on the premises appropriately inaccessible to children Exit interview was conducted with Ms. Ellam via telephone prior to final printing this report. Ms. Ellam stated her understanding of the content of this report and that she will have all corrections listed above completed no later than 10/25/2022. A follow-up visit will be conducted on a later date to ensure the above corrections have been made. LPA provided and reviewed all Provider Information Notice's regarding Lead Testing to Ms. Ellam via email. Ms. Ellam stated her understanding of the information and what is needed to be done by January 1, 2023. A preschool license for 37 children (with no more than 11 toddlers included) may be granted upon receiving the above corrections and final file review. Ms. Ellam stated her understanding. This report shall be kept available at the facility for public review for 3 years. An updated fire clearance will be requested to reflect updated capacity. A copy of this report and appeal rights will be provided to Ms. Ellam via email by COB today. Ms. Ellam stated her understanding that she must respond to the email confirming receipt, as this form of receipt replaces her signature.
