What the inspector wrote

On 12/29/22 at 10:20am, Licensing Program Analysts (LPA) Samantha Clenista and Gerald Poindexter conducted an announced Pre-licensing follow-up inspection. Upon arrival, LPAs met with Facility Representative, Alexandra Ellam. Purpose of this visit is to inspect the newly installed playground structure that Ms. Ellam placed in the preschool playground. Ms. Ellam initially sent pictures to LPA of the structure via text message on 11/07/22, however the structure looked high and not age appropriate. LPA waited for age appropriateness verification from Ms. Ellam. Facility is requesting a license to care for children ages 2-5 years old. During inspection, LPAs measured the various dimensions of the play structure and will consult with management and review the U.S Consumer Product Safety Commission- Public Playground Safety Handbook to verify if the structure (or what specific areas of the structure) is age appropriate. In the meantime, Ms. Ellam has placed caution tape around the entire structure and has provided alternative play equipment for the children to use until the structure has been approved. LPA informed Ms. Ellam that the structure cannot be used by any children until the department can confirm the age appropriateness of it and approves it. Ms. Ellam stated her understanding and will only have the children utilize the other play equipment/activities such as balls, parachute, and tricycles until then. LPAs also observed a nut missing on top of the tire swing and an exposed wooden plank that could cause splinters with a small gap. During inspection, LPAs observed the water fountain that is located on site to have standing water of about 3 inches high. Ms. Ellam emptied the extra water so that there is less than an inch of standing water with the rocks and turned off the water fountain so that it stops accumulating. Ms. Ellam stated that the water fountain will be kept off until the church and her establish a more permanent solution to make it inaccessible to children is made. Ms. Ellam stated she will provide LPA Clenista with proof of nut installed and removable/corrected wooden plank via pictures no later than 12/30/222. After LPA Clenista receives the corrections from Ms. Ellam and conducts a final file review, a preschool license for 37 children (with no more than 11 toddlers included) may be granted. Exit interview was conducted with Ms. Ellam.
