What the inspector wrote

*Due to a computer malfunction, this report was not completed during inspection. See file for acknowledgement receipt. On 10/19/2022 at 11:31am, Licensing Program Analyst (LPA), Samantha Clenista conducted an announced Pre-Licensing inspection. Upon arrival, LPA met with Facility Representative, Alexandra Ellam. LPA took a tour of the facility per facility sketch. This facility houses a preschool, toddler and infant program. The Infant Program (children ages 6 week-24 months) will operate in Room 203. Fire clearance was granted on 10/11/2022. If medications are on site, Ms. Ellam stated that will be stored in the director's office in a locked container. All indoor and outdoor activity space utilized for the children were inspected and measured today. The indoor play area used for the infants was observed to be physically separate from the other programs. Room 203 measured at approximately 330 sq. ft., sufficient for only 9 children. LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. There is one changing table within an arm’s reach of a sink however is being used as part of the napping areas fencing. Infant needs and services plan will be located in the classroom, and Ms. Ellam is reminded that they to be updated quarterly. There were no cribs in the napping area during inspection. Ms. Ellam stated that she purchased 6 cribs that are scheduled to arrive by end of week. Also present were sleeping cots for the older infants that can climb out of a crib. LPA reminded Ms. Ellam that each infant's bedding shall be used solely per infant and not shared and shall be taken home at least weekly for washing. All storage containers or trash cans containing solid or diaper waste have tight fitting lids and are in good repair. Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPA reviewed the updated LIC311A with the facility representative today and reminded to keep anything that reads, “Keep Out of Reach of Children” inaccessible to children. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Prior to licensure, the following must be submitted/completed: Label all bathrooms do designate which bathrooms are used for children versus adults Obtain a first kid Submit and obtain an approved playground waiver to allow the toddlers to share the infant playground Purchase/install more play equipment for the infant/toddler Post all required documents for the parents and employees Ms. Ellam stated she will purchase/install refillable water stations as a form of accessible water to children inside and outside the classroom Make the water fountain located on the premises appropriately inaccessible to children Place sufficient amount of cribs in the napping area Make the fencing surrounding the napping area sound proof and fully enclosed Move the changing table so that it is not part of the napping areas fence There is one separate bathroom that consist of 1 toilet and 1 sink which s used by staff and is also designated for use by children who are ill. The director's office is the isolation area for children who are ill. There is a carbon monoxide detector on site located is room 103 and 201. The kitchen area currently includes a sink, two ovens, a stove, a microwave and one refrigerator. Area was was observed free of rodents and/or vermin. Sign-in sheet will be electronic using the application called Bright Wheel. Ms. Ellam stated there are no firearms/weapons on site. LPA observed a water fountain located on the premises and it not appropriately inaccessible to children. The outdoor playground area was inspected and measured. Area was measured at approximately 3,747 sq. ft., which will accommodate no more than 49 children at one time. Facility requested to share this playground with their toddler program children at different times. A playground waiver shall be submitted and approved prior to licensure. When LPA inspected the outdoor playground, LPA observed a big canopy used for shade. Play area has 3 play houses, 1 car and a couple push toys. Area under the shaded area has sand. There is no play or climbing structure present. The playground was observed to be fully fenced (above 4ft. high). Ms. Ellam stated that drinking water will be readily accessible inside and outside the classroom via water bottles/cups and refillable water stations, however, LPA did not observe any on site. Ms. Ellam is reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Incidental Medical services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA and applicant discussed California Megan's Law and LPA provided: www.meganslaw.ca.gov . Exit interview was conducted with Ms. Ellam via telephone prior to final printing this report. Ms. Ellam stated her understanding of the content of this report and that she will have all corrections completed no later than 10/25/2022. A follow-up visit will be conducted on a later date to ensure the above corrections have been made. LPA provided and reviewed all Provider Information Notice's regarding Lead Testing to Ms. Ellam via email. Ms. Ellam stated her understanding of the information and what is needed to be done by January 1, 2023. A license for 9 infants (ages 6 weeks thru 24 months) may be granted upon receiving all pending corrections and final file review. Ms. Ellam stated her understanding. This report shall be kept available at the facility for public review for 3 years. An updated fire clearance will be requested to reflect updated capacity. A copy of this report and appeal rights will be provided to Ms. Ellam via email by COB today. Ms. Ellam stated her understanding that she must respond to the email confirming receipt, as this form of receipt replaces her signature.
