What the inspector wrote

On 9/26/24 at 1:30 PM, LPAs Gerald Poindexter and Renita Rodriguez conducted an unannounced case management inspection while visiting for another matter. LPAs met with Pedro Ontiveros, site director and explained purpose of visit. Present in the facility were 38 daycare children with nine staff and 4 rooms (one room unused). Facility is within ratio and capacity. Upon staff records review, LPAs determined that one of 17 staff was fingerprint cleared, but not associated to the facility. A Type B deficiency was cited today. Type B deficiencies if not corrected pose a potential risk to the health, safety and personal rights of children in care. Exit interview was conducted with Pedro Ontiveros. LPA reviewed this report and copy was provided today. A copy of appeal rights was also given today.Notice of site visit was provided and observed to be posted.
