What the inspector wrote

On 4/14/25 at 1:32 p.m., LPA Renita Rodriguez made an unannounced visit to follow up on self reported child incident reports, received on 11/20/24. Upon entry, LPA met with Director Pedro Ontiveros and explained purpose of the visit. Ratios observed 26 children and 6 staff. LPA conducted interviews, made a confidential names list, observed the playground, and received a copy of the children’s roster. Staff S1 observed child C1 climbing up a structure. As the child was climbing up the hard plastic structure, child hit their mouth. S1 saw blood on the mouth. S1 looked inside child's mouth and did see a chipped tooth. Parent was notified and picked up child from the facility. Child C1 received medical attention by dentist on the same day as incident. Child returned to the facility the next day without medical directives. A second separate incident occurred on the same day at facility Staff S3 observed child C2 going down a tandem slide with another child while holding hands. Child C2 starting to slide down while other child did not. Child holding C2's hand did not let go of C2 hand as child went down slide. C2 arm was pulled in the process by other child, resulting in injury to the arm. Parent of C2 was notified and picked up child C2. Child received medical attention by pediatrician on the same day as incident. Child C2 returned to the facility the next day without medical directives. No deficiency cited. Exit interview conducted and report was reviewed with the Director Pedro Ontiveros. A notice of site visit was given and must remain posted for 30 days.
