What the inspector wrote

On 09/19/2023 at 9am, Licensing Program Analyst (LPA), Samantha Clenista conducted an announced Change of Ownership Pre-Licensing inspection. Upon arrival, LPA met with the following: Christine Alderman (Center Director), Jacqueline Cleumas (Assistant Director), Lona Seitz (Children\342\200\231s Paradise COO), and Diane Prospero (Children\342\200\231s Paradise Director of Child Development). All indoor and outdoor activity space utilized for the children were inspected today. Facility is requesting to be licensed for 41 infants (ages 0-24 months old) in Room 3 and Room 4. Fire clearance was granted on 08/29/23. This facility houses an infant, toddler, preschool and school-age program. Areas used for the infant program were observed to be separate from other programs, both indoors and outdoors. LPA informed facility representatives that the programs shall never commingle at any given time and that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor play. Medications on site will be stored and located in each classroom in an upper cabinet, out of children\342\200\231s reach. LPA continued to tour the facility and observed that Room 3 and 4 are not fully set up in the way Children\342\200\231s Paradise ownership is wanting to take it over. There appears to be a crib/napping area located in each classroom, however neither is fully separated from the activity space (via 4ft. sound absorbing material). After facility representatives alter the space, LPA will conduct a follow-up inspection to measure the area to see how much the activity space accommodates. There is a children\342\200\231s bathroom located between Room 3 and 4 that consists of a two changing tables, one being within arm\342\200\231s reach of a sink and an additional toilet available for potty training children. LPA observed all cribs that were in Room 3 and 4 to be the pull down type, which is no longer allowed. Facility shall obtain approved infant cribs. LPA observed all indoor play space to be complete with age-appropriate furniture and equipment, including but not limited to tables, chairs, cubbies, bookshelves, and other activity supplies for the children. There is a refrigerator for children\342\200\231s use. Ms. Seitz is reminded that all infant bottles shall have their name and date on them. There are napping cots/mats for the older infants who can climb out of a crib. Infant needs and services plan will be stored in the classroom in a binder per child and Ms. Seitz is reminded that they have to be updated quarterly. There is a separate staff bathroom located in the hallway that consists of 3 toilets and 3 sinks. The Director's office and staff restroom will be the isolation area for children who are ill. Kitchen area and food storage area was observed free of rodents and/or vermin. There is an operational carbon monoxide detector on site located in each classroom. The sign-in/out area is electronic (via Care Connect) and will be located in the front lobby area. The facility has two fenced in playground areas located behind and right of the building. LPA informed Ms. Seitz that children must be escorted to and from the playground, and that staff must maintain direct supervision at all times. The infant outdoor play areas measured at approximately 1,531.64 sq. ft., sufficient for only 20 children at one time. Area was observed to be fully fenced in, however had sharp succulents in the area. Area had canopies used for shade. Area had age appropriate toys and play equipment. Drinking water is readily accessible inside and outside the classroom via refillable water fountains, faucets and refillable water bottles. Ms. Seitz is reminded that any changes to the facility must be reported to and approved by Community Care Licensing. LPA reviewed with Ms. Seitz the LIC 311A, Records to Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. LPA discussed the safe sleep regulations with Ms. Seitz and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep , as an additional resource. LPA also informed facility representatives of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Ms. Seitz was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-carelicensing/subscribe and select the Child Care option to receive email communication. Prior to licensure, the following is needed: set up both Room 3 and 4 with the appropriate designated crib area (area must meet 4ft. high and of sound absorbing material for barrier), post required postings, remove the second changing table that is located in the children\342\200\231s bathroom, or move it so that it is within arm\342\200\231s reach of a sink, remove and replace the drop down cribs with approved cribs, remove or make any hazardous plants and wooden planks inaccessible that is currently located out on the side playground area, submit an updated indoor and outdoor facility sketch and place signs on staff restroom. Exit interview was conducted and report was reviewed with facility representatives. A notice of site visit was given to Ms. Seitz and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. A follow-up inspection is required to dictate the facility\342\200\231s indoor activity space. Ms. Seitz stated her understanding. Ms. Seitz understands that this report shall be kept available at the facility for public review for 3 years.
