What the inspector wrote

On 5/28/2025 @ 10:15AM, Licensing Program Analyst (LPA) Nancy Diaz conducted an unannounced inspection. LPA disclosed the purpose of the inspection and was granted facility entry by Vianney Barraza, Site Director. LPA toured the facility with Mrs. Barraza. Observed present today were 28 preschool and 20 infants in the following rooms: Orugas room with 12 infants and staff Cynthia Orellana, Lidia Luevanos & Crismeglis Bermudez Capullo room with 8 toddlers and staff Alejandra Zatarain & Daniella Godinez Mariquitas room with 10 preschool children and staff Linda Rocha Mariposas room with 11 preschool children and staff Jessica Payan, Mariana Garcia & Valeria Munoz Abejas room with 7 preschool children and staff Ashley Torres A review of staff records on this date indicates that one staff Mariana Garcia was not associated to the facility. The licensee has not exceeded the conditions, limitations and capacity specified on the license. Site Director (Vianney Barraza) and a few of her staff are CPR/First Aid certified. Current certifications were on file. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are inaccessible. Medications are kept in a safe place, inaccessible to children. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. All toilets, handwashing facilities are in safe and sanitary operating condition. All floors are clean and safe. The child care center was observed to be clean, safe, sanitary and in good repair to ensure the safety and well-being of children, employees and visitors. Facility maintains a carbon monoxide detector that meet the standards established in Chapter 8 of Part 2 of Division 12. CONTINUED ON LIC809C & LIC809D Infant changing tables have a padded surface not less than one inch thick. Infant changing tables have raised sides at least three inches high. Toys are observed to be safe, and did not have sharp points or edges or splinters or made of small parts that can be pulled off and swallowed. Facility maintains sufficient napping equipment. The infant facility have indoor/outdoor activity space that is physically separate from space used by preschool children. Direct visual supervision are provided by staff at all times. Infant individual feeding and needs and services plans are maintained. Nap documentations are maintained that included: infant\342\200\231s name, date, time of each 15-minute check and initials of the person who conducted each check. The kitchen, food-preparation and storage areas are kept clean, free of litter. Food are protected against contamination. All storage containers for solid waste have a tight-fitting covers that are kept on and in good repair. Playground equipment were observed to be in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space are maintained in safe condition and free of hazards. The areas around or under high climbing equipment, slides, and similar equipment was cushioned with material that absorbs a fall. Children were observed to be under the supervision of qualified staff. An isolation area has been designated for children who becomes ill during the day. Children\342\200\231s records were reviewed today. All required forms were on file. Menus are posted in a place visible by the child\342\200\231s authorized representative. All children are signed in/out by a representative who uses a unique PIN code and has recorded the time of day. Child\342\200\231s record also contain a medical assessment. Staff records reviewed today contain a health screening as required by the regulation. Staff have completed the mandated reporter training (effective 3/2018). This training shall be renewed every two years. All staff have immunization record indicating that they have been immunized against influenza, pertussis and measles. CONTINUED ON LIC809C & LIC809D This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP . Site Director, Vianney Barraza was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Select \342\200\234Receive Important Updates\342\200\235 then put the email address in and choose which program(s) you would like to subscribe to and select \342\200\234subscribe.\342\200\235 LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. CONTINUED ON LIC809C & LIC809D Site Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Exit interview conducted and report was reviewed with facility representative, Vianney Barraza. A copy of this report, along with Appeal Rights (LIC9058), were provided. A notice of site visit was given and must remain posted for 30 days. LPA observed that the notice of site visit was posted during the inspection. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. TYPE B DEFICIENCIES WERE CITED TODAY. CIVIL PENALTY WAS ASSESSED. Type B deficiency if not corrected poses a potential risk to the health, safety or personal rights of children in care.
