On 09/22/2025 at 1:00PM, Licensing Program Analyst (LPA) Hanna Lucas made an unannounced case management visit to follow up on a self reported incident that occurred on 09/16/2025 wherein the Director, Cecilia Baeza, was told by a staff member(S1), that they had pulled a child(C1) by their arm and left a mark. The incident did not require medical treatment. There were 63 children with 16 teachers, in 8 classrooms, during today\342\200\231s visit. At the time of the incident, there was 3 staff members(S1-S3) present, with 12 children, in Classroom #5. (S1) told the Director that (C1) was not listening when asked to put a toy away, (S1) then grabbed the child's arm harshly to re-direct. The arm grab resulted in a red mark. The parents were notified the same day. (S1) was placed on administrative leave after the incident occurred, and then was terminated on 09/19/2025. LPA inspected the facility, interviewed staff members, and collected pertinent documentation. Exit interview conducted with the Director, Cecilia Baeza. Appeal rights were provided. A Notice of Site Visit was provided and must remain posted for 30 days. (1) TYPE B is issued on the attached LIC 9099-D.
