On 1/12/26 at 10:45 AM, Licensing Program Analyst (LPA) Gerald Poindexter made an unannounced Plan of Correction (POC) inspection related to the deficiencies cited on 12/19/25. The POC due date for the deficiencies was 12/29/25 and 1/5/26, which, at the time, was explained to and acknowledged by interim director Tomi Kelly. Observed present today were 2 infants and 3 preschoolers with staffer Katie Brown. LPA met with Ms. Kelly and the following deficiencies cited on 12/19/25 were reviewed: \302\267 Infant Care Center Director Qualifications and Duties, 101415(b) \302\267 Teacher Qualifications and Duties, 101216.1(c)(1) \302\267 Reporting Requirements, 101212(b) \302\267 Limitations on Capacity, 101161(a) To date, the LPA notes that corrections have not been provided for these deficiencies. Ms. Kelly stated that she believed that the due date was later this month in January 2026, and was not aware that the dates had past. LPA explained to Ms. Kelly that failure to correct the deficiencies by the POC due date can result in a civil penalty of $100 per violation, per day until the violation is corrected. LPA P oindexter i nformed Ms. Kelly that this report dated 1/12/26 documents and assesses four (4) civil penalties. Please be advised that FAILURE TO PAY the required civil penalty payment may result in the REVOCATION OF YOUR LICENSE. You must respond within 30 days with the payment of or a proposed payment plan that includes the first payment. Further, the Department will not approve any requests for increase in capacity or for additional capacity of additional licenses while civil penalties remain unpaid. Exit interview was conducted with Tomi Kelly, interim director. LPA reviewed and provided a copy of the licensing report. Appeal rights provided. A Notice of Site Visit was provided and must be posted for 30 days.
