What the inspector wrote

On 1/26/26 at 2:30 PM, Licensing Program Analyst (LPA) Gerald Poindexter made an unannounced case management visit while at the facility on another matter. LPA met with Rachelle Wise, director of the Rising Star Fairmount location and facility representative, as designated by the licensee. Observed present today were 2 infants and 2 preschoolers with qualified teachers Belinda Thomas and Juana Alonso. During this visit, LPA notes that: \302\267 Ms. Wise stated that Ms. Alonso was hired 1/20/26, but when asked by the LPA, her personnel file could not be located. LPA observed Ms. Alonso\342\200\231s educational transcript (qualified teacher) and that she is background cleared and associated to the facility. \302\267 Based on record review, LPA also noted that Ms. Wise is not associated to the facility as required. LPA, upon entry to the facility, observed Ms. Wise supervising children in care. \302\267 Ms. Wise stated that she was designated as facility representative by the licensee. However, her status at the facility was not communicated to the Department and the facility has not designated a fully qualified teacher to act in the absence of a director. See LIC809D for deficiencies cited. Exit interview was conducted with Rachelle Wise, facility representative. LPA reviewed and provided a copy of the licensing report with Ms. Wise. Appeal rights provided. A Notice of Site Visit was provided and must be posted for 30 days.
