What the inspector wrote

On 09/08/2025 at 1:30PM, Licensing Program Analyst (LPA) Hanna Lucas made an unannounced case management visit to follow up on a self-reported incident that occurred on 08/28/2025 wherein a child (C1), was showing a staff member (S1), some break dance moves, and ended up falling onto their right arm. The fall resulted in injury, and did require medical treatment. There were 33 children with 16 teachers, in 4 classrooms, during today\342\200\231s visit. LPA toured the classroom where the incident occurred, interviewed staff members involved, reviewed, and collected pertinent documentation . At the time of the incident, there were 2 fully qualified teachers, with 4 children present in Classroom #1. (C1) was observed by (S1), dancing on the circle time rug, and then threw themself onto the ground, landing on their right arm. The Director, Heide Fields, notified the parents immediately and the child was picked up from the facility within 10 minutes. (C1) returned to school the following week, with normal behavior and no further concerns. Facility had proper supervision in place, responded appropriately, and notified the parents promptly. No deficiencies are cited. Exit interview conducted with the Director, Heide Fields. Appeal rights were provided. A Notice of Site was provided and must remain posted in public view for 30 days .
