What the inspector wrote

The Director, Eva Campbell, provided the facility’s menu for the month of July, and the LPA observed that it does not meet nutritional requirements, per Title 22 regulation. The preponderance of evidence standard has been met; therefore, the above allegations are found to be SUBSTANTIATED. (3) TYPE B deficiencies are cited on the attached LIC 9099-D and the facility has been informed that the existing waiver may be rescinded upon further review. An exit interview was conducted with the Center Director, Eva Campbell. LPA provided a copy of the appeal rights along with a copy of this report.
