What the inspector wrote

On 9/29/25, Licensing Program Analyst (LPA) Saraliz Velando conducted an unannounced visit for a complaint received on 7/25/25. The LPA met with the Director, Sandra Reyes. There were 37 children and 12 staff present today. Based on the information obtained from file review, parent and staff interviews, it was determined that staff restrain infants in high chairs. The interviews revealed sufficient information to prove that staff members restrain infants in high chairs to prevent a child from biting others and high chairs should only be used for the purpose of feeding. The preponderance of the evidence has been met and therefore, the above allegation is found to be SUBSTANTIATED. Type B Violation was cited. Refer to the next page LIC 9099-D for deficiency. The exit interview was conducted with the Director, Sandra Reyes. The Appeal Rights and a copy of the licensing report were provided. A notice of site visit was posted and must remain for 30 days.

This requirement was not met by: Based on interviews, staff members were witnessed placing a child in a high chair to prevent biting to other children. This poses a potential health, safety or personal rights risk to the children in care.
