What the inspector wrote

On 9/29/25, Licensing Program Analyst (LPA) Saraliz Velando conducted an unannounced case management inspection to re-add Rooms #1 & #2 due to flood damage that originally occurred on June 2, 2025. Present today were 37 children and 12 staff. Fire clearance was originally granted on 03/26/25 for Rooms 1 and 2. Licensee is requesting to re-add Rooms #1 & #2 for Preschool ages 2-5, to the facility after a flood that occurred in June 2025. LPA inspected the two rooms and observed there were no changes made, most repairs have been completed. The Director, Sandra Reyes will submit proof of the following corrections when completed: 1. Baseboards in Rooms 1 and 2. 2. Thresholds for Rooms 1 and 2. 3. Reorganize furniture in Room 2. 4. Remove construction tools from restrooms of Rooms 1 and 2. The Director, Sandra Reyes will notify the department once completion of repairs in Rooms #1 & #2. The Director understands these corrections should be completed within 30 days of this report. Exit interview was conducted and LPA Velando reviewed and provided a copy of the licensing report. Notice of site visit was given and must be posted for 30 days.

This requirement was not met by: Based on interviews, staff members were witnessed placing a child in a high chair to prevent biting to other children. This poses a potential health, safety or personal rights risk to the children in care.
