What the inspector wrote

On 12/4/25 at 8:50AM, Licensing Program Analyst (LPA) Annette Sutherland conducted an unannounced Case Management inspection to follow up on a lead testing result for one of the classroom sinks at the facility, which showed an Action Level Exceedance (ALE). LPA met with the facility director, Sandra Reyes, to discuss the findings and next steps. Water outlet reported with 5.5 ppb or greater lead exceedance level: Classroom sink- Sample B (9.3 Ug/L or ppb) LPA observed the sink with a do not drink sign. The sink has a connector for drinking water that has also been made inaccessible for drinking until it is retested. All other fountains and water outlets tested within an acceptable level with no lead exceedance. Parents have been notified by letter and results were posted. See LIC 809D for Type B deficiency cited during today's visit. An exit interview was conducted with facility representative, Sandra Reyes . A copy of this report was provided along with a Notice of Site Visit that must be posted for 30 days.
