What the inspector wrote

On 01/07/2026 at 9:30am, Licensing Program Analysts (LPA), Samantha Clenista and Selina Siao conducted an announced Pre-Licensing inspection to add an infant program and to consolidate their existing license (376105007) into a single license. Upon arrival, LPAs met with Facility Representatives, Saied Nakhai (CEO) and Heather Davis (Center Director). Facility is requesting to be licensed for 10 infants (6-24 months) in the Infant Care Room, 52 toddlers (18-36 months) in the Jellyfish, Octopus and Otter Room, and 100 preschoolers (2.5 to Kinder) in the Seahorse, Starfish, Whales and Sea Turtles Room. Hours of operation will be Monday thru Friday, 7:30am \342\200\223 5:30pm. All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed Facility Representatives that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities to ensure their overall health and safety. When medications are on site, Director stated that they will be in red emergency bag located in both the upper and lower offices. A fully equipped first aid kit is located on the playground. There is an operational carbon monoxide detector on site located in each room. All required licensing documents were observed posted in both offices. Children will be signed in and out via Procare. LPAs continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space for infants measured at 948.50 sq. ft., which is sufficient to accommodate the requested capacity of 10 infants. Total indoor activity space for toddlers measured at 2,106 sq. ft., which is sufficient to accommodate the requested capacity of 52 toddlers. Total indoor activity space for preschoolers measured at 3,634.75 sq. ft., which is sufficient to accommodate the requested capacity of 100 preschoolers. LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots/mats, cribs, bookshelves, and other activity supplies for the children. Infant toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water is available inside and outside the classrooms via Brita filter water jugs and water cups/bottles required by parents/guardians to bring. LPAs observed all hazardous items to be inaccessible to children. There are no bodies of water on the property. Facility has an armed guard that is on the property during hours of operation. A waiver to request that this armed guard be present during the hours of operation shall be submitted and approved prior to licensure. Fire clearance was granted on 12/09/2025. The infant room has low chairs that have belts on them for feeding, which have broad-based legs, plastic seats are in good repair. The seats for the feeding chairs are made of washable, moisture-resistant material. There is a changing table within arms reach of a sink and has at least 1\342\200\235 padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3\342\200\235 high. Facility Representatives are reminded that the diapering sink shall not used for meal preparation or dishwashing. Cribs meet regulatory requirements. There are six cribs (all of wheels) in the crib area, however is not separated from the activity space via 4ft. sound absorbing barrier. Sleep logs will be kept via Procare. Needs and Services Plans and Sleep Plans (LIC9227) will be kept in a binder located in the classroom, and Facility Representatives were reminded that they have to be updated quarterly. LPAs observed a total of 4 sinks, 2 toilets, 1 potty chair available for infant use. LPAs observed a total of 10 sinks and 6 toilets available for toddler use. LPAs observed a total of 13 sinks and 7 toilets available for preschool use. These are sufficient to accommodate the requested overall capacity for each program. There are separate staff restroom (one located on the upper and lower building). The isolation area for children who are ill will be the upper office and a restroom located next to the director\342\200\231s office. Facility Representatives reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Facility provides only snacks. The kitchen area currently includes microwave, toaster, double sink, 2 dishwashers, oven and refrigerator. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. The facility currently has a fully fenced playground area for each program (infant, toddler and preschool). Fencing (via chain link, metal security fence) is at least four feet high. The total square footage for the designated infant playground measured at 549.45 sq. ft., which can only accommodate 7 infants at a time. A playground waiver to share the infant playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure). There are two toddler playground areas (Toddler 1 and Toddler 2 Playground) which in total measured at 2,106.82 sq. ft. which can only accommodate 28 toddlers at a time. There are two preschool playground areas (Preschool 1 and Preschool 2 Playground) which in total measured at 11,980.14 sq. ft., which is sufficient to accommodate the requested 100 preschool children capacity. Shade is provided via trees, gazebos, pergolas, and patio covers. There are sufficient outdoor age-appropriate toys and play equipment available on all the playgrounds. There is adequate cushioning in fall zones of climbers provided by turf and rubber pour and play. LPAs observed all hazardous items on the playgrounds to be inaccessible to children. Facility Representatives were reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Facility Representatives were reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. This facility plans to provide Incidental Medical Services \342\200\223 IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPAs reviewed with Facility Representatives the LIC 311A, Records to be Maintained at the Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Entrance Checklist was provided to the applicant. LPAs discussed the safe sleep regulations Facility Representatives and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep , as an additional resource. LPAs also informed Facility Representatives of the importance of checking for and removing recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Facility Representatives were informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. A notice of site visit was given to Facility Representatives and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with Center Director, Heather Davis. Barrier should be at least 4ft. high and made of sound absorbing material. Update arial facility sketch to include Infant Playground, identify upper and lower offices, and location of isolation area/restrooms Update indoor facility sketch for Infant Care Room to reflect where the crib area is located. Submit a waiver request to allow an armed guard on site. Due to limited square footage for both the infant and toddler playgrounds, a playground waiver request to share the infant and toddler playground areas at staggered times to allow no more than 7 infants on the infant playground and no more than 28 toddlers on the Toddler 1 and Toddler 2 playground combined shall be submitted. Facility Representatives understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.
