On date and time listed, Licensing Program Analyst's (LPA's) Linda Almaraz and Jeanette Sanchez arrived at the facility to conduct an annual inspection as part of a compliance review. The licensee has also requested to increase the capacity to that of a Large Family Child Care Home which will be reviewed today as well. LPA's toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed: Fire Clearance was granted on 3/25/22. \302\267 Normal days and hours of operation are: Monday - Friday from 7AM through 5PM. Ages served 6 month to 5 year olds. \302\267 Off-limit areas include: All second floor, Garage, Kitchen, left and right side of backyard. \302\267 The facility is not operating within the licensed capacity and appropriate ratios. During the visit there was 9 children present and 4 infants and 5 children under the age of 4 years old. \302\267 Appropriate supervision was not provided during this inspection \302\267 A working telephone is present and the current number is on file \302\267 Appropriate fire extinguisher, smoke detector and carbon monoxide detector present and were tested by the Licensee during this inspection. \302\267 The Fireplace is covered inaccessible to children \302\267 All hazardous items were not stored inaccessible to children \302\267 Toxins were not locked (Continued on an LIC809-C) \302\267 Weapons are not present in the home. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations \302\267 Stairs are barricaded \302\267 Clean, safe and age appropriate toys \302\267 The facility does not have a current roster on file \302\267 Facility Sketch, Emergency Disaster Plan and Notification of Parent\342\200\231s Rights poster are posted \302\267 Documentation of fire and disaster drills were not on file due to the Licensee not conducting the drills \302\267 The spa located in the backyard had an unlocked cover accessible to children in care. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Verification of control of property on file \302\267 Children\342\200\231s records were not complete. All infants were missing sleep log and an infant was missing a sleeping plan. \302\267 Employee\342\200\231s records were not complete \302\267 Mandated Reporter Training completed on 9/24/2020 \302\267 Pediatric CPR and First Aid Card expire on 7/25/2022 \302\267 Health & Safety Certificate - Completed on 6/1/2020 \302\267 Residents and staff records were reviewed and all adults who require caregiver background checks have received all required clearances and/or exemptions. (Continued on an LIC809-C) The licensee confirmed that there are no Registered Sex Offenders living in the facility and/or using the facility address for their mailing address. The facility is not providing IMS services: Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. You will receive an invoice in the mail. Do not send money until you receive your invoice. Do not send cash. See LIC809-D for cited deficiencies. Immediate civil penalties assessed. Once proof of corrections is submitted the Capacity increase for a Large family child care license will be submitted. During the inspection LPA's went over the Pre-licensing handbook to ensure she understands regulations. A hard copy of the handbook was provided along with an electronic copy via email.
