What the inspector wrote

On the date and time listed, Licensing Program Analysts (LPA) Hayley McCarthy arrived at the facility to conduct a Case management incident follow-up visit on an Unusual Incident Report (UIR) received by the Department on 04/16/25. LPA met with Anamaria Zamudio (Assistant Director) to discuss the incident. A tour of the facility was granted, and census was conducted. On 04/15/25, it was self reported that child 1 (C1) had been playing with toy trains when he stood up to get a piece and tripped on a train falling face first onto the edge of a toy tree house nearby. Staff 1 (S1) observed the incident and immediately assisted C1. S1 applied pressure to the the wound with a towel and ice pack until the bleeding stopped. C1's mother was notified and arrived shortly after and took C1 to urgent care where he received two staples. On 04/22/25, C1's staples were removed with no further complications and C1 returned to the center. LPA met with assistant director and discussed the incident. During inspection, LPA observed toy tree house that C1 fell on and determined that there were not sharp edges or other safety concerns with the toy. LPA has determined that the facility took the necessary steps to ensure the health and safety of the children in care. Based on the information gathered, there appears to be no violations of Title 22 Regulations found at this time, and therefore, there were no deficiencies cited during this inspection. An exit interview was conducted, and a copy of this report was provided to Anamaria Zamudio (Assistant Director). A Notice of Site Visit was issued and the Licensee understands that it must remain posted for 30 days.
