    Limitations on Capacity:(a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation.This requirement was not met as evidenced by, Based on LPAs observations it was witnessed on 2/19/26 2 preschool-aged children in the infant room. Based on interviews conducted it was stated that children from the 2 year old classroom will sometimes be in the infant room in the morning until the next teacher arrives. This is a potential health and ssafety risk to children in care.

This requirement was not met as evidenced by, Based on video observation and interviews conducted, cots were propped up against the wall and fell onto C1 resulting in a head injury that required medical attention. This is a potential healthand safety risk to children in care.

This requirement was not met as evidenced by, Based on interviews conducted, it was disclosed that S1 would mock the children and toss items such as plates towards them on the table during meal time. This is a potential health and safety risk to the children in care.

    Personal Rights: (a) The licensee shall ensure that each child is accorded the following personal rights: (3)free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. This requirement was not met as evidenced by, Based on interviews conducted it was disclosed that S1 has been observed to pulling children by their arm in a rough manner and tugging on their hair to move them into line and has been observed and overheard yelling or raising voice in classroom towards children. This is a potential health and safety risk to the children in care.
