On May 20, 2026, at 3:15 PM, Licensing Program Analyst (LPA) William Chancellor conducted a Case Management inspection at Discovery Preschool Vista (CCC). LPA was greeted by Director (DIR) Jen Willson, where LPA provided the reason for the visit. During the inspection LPA was provided a tour, took census, requested relevant documentation and interviewed four staff. A case management visit is being conducted in response to the receipt of an Unusual Incident Report (UIR). CCC self reported an incident occurring on May 8, 2026, involving child 1 (C1) being left on the playground during a transition indoors. Four staff (S1-S4) involved with the incident and present in the day in question were interviewed during today’s visit. Interviews corroborated that during a transition from outdoors to indoors, C1 was not included in the count and remained outside for less than one minute. At the same time, a toddler teacher (S2) was outdoors on a neighboring playground and observed C1. Record review verified that C1 parents were notified in a timely manner and staff were trained on the appropriate transition protocols. LPA determined that there was not a lack of supervision, where C1 was immediately transitioned indoors by S2. Based on information gathered, the facility acted appropriately, and there are no deficiencies cited at this time. An exit interview was conducted, and a copy of this report was provided to Licensee Sally Chenoweth. A Notice of Site Visit was provided and must remain posted for 30 consecutive days.
