What the inspector wrote

On November 1, 2023, at 11:37 am, Licensing Program Analysts (LPAs) Cindy Hamilton and Amber Shaw arrived at the facility on a case management inspection. LPAs met with Interim Director/Owner Maria Hernandez and provided purpose of inspection. At the time of inspection, LPAs toured the facility and took census. LPAs observed, the facility has flying insects in 3 year olds ladybugs classroom and in the kitchen. In the Ladybugs c classroom, LPAs observed a cup with several flying insects on and flying around the cup. LPAs also observed bulging, water stained, and possibly mold/mildewed ceiling tiles in the pre-k Grasshopper classroom in which the interim director Maria Hernandez advised that the air conditioner is leaking and is still being used. In the kitchen, LPAs observed food on the counters with several insects flying around. Some of the food was covered and some of the food was not covered. A Type B citation for Building and Grounds was issued, see LIC 809-D. An exit interview was conducted with Licensee and a copy of this report, appeal rights and notice of site visit was provided. Director was reminded that the notice of site visit must remain posted for 30 consecutive days or civil penalties may be cited.

This requirement was not met as evidenced by: Based on observation, the facility is not in good repair, the ceiling tiles in Grasshoppers room are bulging, water stained and possibly mold or mildewed due to leaking Air Conditioning unit which poses a possible health, safety and/or personal rights risk to children in care.

This requirement was not met as evidenced by: Based on observation, the facility has a large amount of flying insects in the Grasshopper and Lady Bugs classrooms and kitchen which poses a possible health, safety and/or personal rights risk to children in care.
