What the inspector wrote

On December 6, 2023 at 10:26 AM , Licensing Program Analyst (LPA) Cindy Hamilton arrived at the facility to conduct a "Plan of Correction" (POC) visit. LPA Hamilton met with owner/director Maria Hernandez. During a previous visit conducted by LPA's Cindy Hamilton and Amber Shaw on 11/01/23, the facility was issued citations for Title 22 Section 101238 Buildings and Grounds. The plan of correction for the citations was to repair and/or replace leaking air conditioner and ceiling tiles, check for mold and have a Pest Control company serve the center. During the POC visit, LPA inspected the kitchen, Grasshopper and Lady Bug classrooms where the air conditioner was suspected to be leaking and flying insects were observed. LPA observed that the ceiling tiles had been replaced and advised it was the roof that was leaking and it was also repaired. LPA observed that one tile had not been replaced that held the fire alarm and was advised that Fire Force would be coming out on December 27, 2023, to replace that tile. LPA also observed that there were no flying insects in the CCC except for a couple in the kitchen area. LPA was advised that the property manager has a contract with Lloyd Pest Control to come out every two weeks. The initial visit by Lloyd Pest Control was conducted on November 13, 2023. Director stated she does not have a copy of the contract but will obtain a copy to provide to LPA to show frequency of future visits. The Plan of Corrections noted on 11/01/23 are being extended at this time until December 13, 2023 for Director/owner to obtain documentation. An exit interview was conducted, a copy of this report, appeal rights and the notice of site visit were reviewed with the Director/owner, Maria Hernandez. Director was reminded that the notice of site visit must remain posted for 30 days.

This requirement was not met as evidenced by: Based on records review, on 10/27/23 the chainlink fence surrounding the perimeter of the CCC was left open which poses a potential health, safety risk and/or personal rights risk to children in care.
