Correction check·December 22, 2023

On December 22, 2023 at 9:45 AM, Licensing Program Analyst (LPA) Cindy Hamilton arrived at the facility to conduct a "Plan of Correction" (POC) visit. LPA Hamilton met with owner/director Maria Hernandez. During a previous visits conducted by LPA's Cindy Hamilton and Amber Shaw on 11/01/23 the facility was issued citations for Title 22 Section 101238 Buildings and Grounds. The plan of correction for the citations was to repair and/or replace leaking air conditioner and ceiling tiles, check for mold and have a Pest Control company serve the center. During the POC visit, LPA inspected the kitchen, Grasshopper and Lady Bug classrooms where the flying insects were observed. LPA observed that the flying insects were no longer present and was advised by Owner/Director that Lloyd Pest Control came out again on December 8, 2023 after hours and treated the facility. Owner/Director also advised that she has been unsuccessful in obtaining documentation regarding the pest control for the child care center from the property management company. In addition, LPA was previously advised that Fire Force would be coming out on December 28, 2023, to replace the remaining ceiling tile. The Plan of Correction noted on 11/01/23 is being extended at this time until December 29, 2023 for Director/owner to provide proof of replaced ceiling tile. An exit interview was conducted, a copy of this report, appeal rights and the notice of site visit were reviewed with the Director/owner, Maria Hernandez. Director was reminded that the notice of site visit must remain posted for 30 days.
