Licensing Program Manager (LPM), Stephanie Hudak and Licensing Program Analysts (LPAs), Joanne Domingo and Cindy Hamilton, arrived at the facility for the purpose of conducting a Case Management visit. LPA conducted COVID-19 Screening prior to entry into the facility by the Licensee, Irma Guerrero. Administrator, Daniel Espinosa provided LPM/LPAs guidance via FaceTime. The facility has been operating on a COVID-19 Temporary Waiver to care for school age children since 7/15/2021. LPM, Stephanie Hudak advised Licensee the purpose of the visit was to review facility records and obtain copies of documents that have been pending for the application. An exit interview was conducted and during the interview, Licensee, confirmed that there are no Registered Sex Offenders living in the facility and/or using the facility address for their mailing address. A copy of this report along with a Notice of Site Visit posted was provided to Licensee on this date and time. A copy of this report must be made available to the public for 3 years.
