Facility-requested visit·April 14, 2023

On April 14,2023 at 11:00 AM, a Case Management inspection was conducted by Licensing Program Analyst (LPA) Cindy Hamilton and Licensing Program Manager (LPM) Pauline Beschorner to conduct a safety inspect of the facility Fellowship Hall for temporary relocation of school-age program due to construction on the lower level. There were no children present and no safety hazards present. There was 3 bookshelves present, a supply cart and a small child's table. Per Prospero the children will utilize the tables provided by the Fellowship Hall. Prospero will email a picture of the set up once completed to LPA Hamilton and before use. The fire clearance provided by licensee for fellowship hall is dated 09/24/2020 and is the most current inspection per Diana Prospero provided by the church. An exit interview was conducted and a copy of this report and appeal rights was provided to Propspero on this date. A Notice of Site Visit was issued and posted.
