What Happened
Plan of Correction follow-up visit for deficiencies issued 12/15/25. Director unaware of due dates and has not made progress. First deficiency extension granted to 1/5/2026. Second deficiency (staffing plan) deemed repeat violation with penalties to be issued.
Observations
- Director unaware of Plan of Correction due date of 12/22/25
- Still in process of collecting documents for CCR 101216(g)(1) deficiency
- No staffing plan created for CCR 101229(a) deficiency
- Has not contacted staffing agencies, posted job openings, or conducted staff interviews

(a) The licensee shall provide care and supervision as necessary to meet the children's needs.This requirement is not met as evidenced by: Based on observation, interview and record review, the licensee did not comply with the section cited above where the facility does not have adequate staffing. The CCC has 1 qualified teacher with units, who also acts as the Director, and works open to close, every day of the week, and has not made efforts to hire additional staff, which poses/posed a potential health, safety or personal rights risk to persons in care.
