Inspector’s narrative

What the inspector wrote

On the above date and time, Licensing Program Analyst\342\200\231s (LPA\342\200\231s) Kelly Gerth and Kelli Waters arrived at the Childcare Center (CCC) on a case management inspection, to follow-up on an Unusual Incident Report (UIR) which occurred on February 02, 2026. Upon arrival, LPA\342\200\231s met with Director Britney Golden and provided purpose of inspection. Community Care Licensing (CCL) received a written UIR on 02/11/2026 which stated that after transitioning in from outdoor play, a child complained their head was hurting and appeared lethargic, disinterested in eating lunch or interacting with friends. Staff radioed to Director and simultaneously attended to the child, applying a cold compress to the child\342\200\231s head, while the Director contacted the parent, who arrived approximately 15 minutes later and decided to take the child for medical attention. LPA\342\200\231s found that the moment the child seemed to be in distress, staff attended to the child and contacted the Director and the child\342\200\231s parent right away. During the visits, LPA\342\200\231s interviewed a total of 3 staff, where it was also found that at no time did any staff witness the child fall or injure themselves and the child was not exhibiting any behaviors out of the norm until the moment the child sat down, expressed their head hurting and appeared to be tired. After the incident, the Childcare Center (CCC) continued to follow up with the parent and check on the status of the child. Based on the information obtained during the visit, there appears to be no violations of Title 22 Regulations pertaining to the reported incident and LPA\342\200\231s determined that the facility took the necessary steps to ensure the safety of the children. An exit interview was conducted and a copy of this report was provided to Director Britney Golden. Notice of site visit was provided and must remain posted for 30 days.
