What the inspector wrote

On August 28, 2025, at 9:30 AM, Licensing Program Analyst (LPA) William Chancellor arrived at Children's Choice Academy- Vista (CCC). LPA was greeted by Director (S1) Coralito Garcia, shortly after arrival Area Director (S2) Rosa Mendoza arrived. A case management visit is being conducted in response an Unusual Incident Report (UIR) being called into Community Care Licensing (CCL) on August 15, 2025. The incident involved a child being left unsupervised in the Frog's 1 class restroom. During the visit, LPA conducted interviews with three staff (S1- S3) and requested documents relevant to the incident. Two of two staff interviews present on the day of the incident, confirmed that Frog's classroom transitioned outside at approximately 3:30 PM. Child 1 (C1) was located alone in the class restroom at approximately 3:42 PM. This time frame confirms that C1 was unsupervised for approximately twelve minutes. Based on confidential interviews conducted and records reviewed, the facility is being cited under California Code of Regulations Title 22, Section 101229(a)(1) Absence of Supervision. Please reference 809-D. This is a second incident within a twelve month period, with the last deficiency being cited on July 9, 2025. Due to a repeat violation, a civil penalty of $1000 is being assessed. In addition the facility representative shall post and provide copies of the report to parents/guardians of the children currently enrolled by the next business day and any families newly enrolled within the next year. An exit interview was conducted with area Director Rosa Mendoza. LPA provided copy of this report, including LIC 9099D, LIC 9224, appeal rights, and a Notice of Site Visit was handed to facility representative, Rosa Mendoza.

Based on two of two interviews and records reviewed, C1 was left alone in the class restroom for approximately twelve minutes without supervision. C1 was located when staff passed through the class. Which poses an immediate health, safety and/or personal rights risk to children in care.
