Inspector’s narrative

What the inspector wrote

On December 17, 2025, at 09:31 AM Licensing Program Analyst (LPA) Hayley Corn and Licensing Program Manager (LPM) Deborah Mullen conducted an unannounced Case Management inspection for a capacity change. The center has requested an increase in the center's infant capacity to 28. Upon arrival, LPA and LPM met with Director Karina Cueva. The center is requesting to be licensed for 28 infants (ages months 0 to 24 months) in rooms 2 and 6. The center is currently licensed for 12 infants in room 2. All indoor and outdoor activity space utilized for infants was inspected and measured and toilets and sinks were counted. LPA and LPM observed all outdoor activity space to be complete with safe, age-appropriate play equipment, including shaded coverage. Total outdoor activity space for infants measured 784.13 square feet which allows for a capacity of 10 infants. LPA and LPM informed director that a waiver would need to be requested for an alternating schedule on the playground so that no more than 10 infants are on the playground at one time. Total indoor activity space for infants measured 846.34 square feet. The facility has 2 potty chairs and needs a total of 5 potty chairs to meet the requirements for the requested capacity of 28 infants. During the visit, director purchased 3 additional potty chairs to meet the requirement. There are 2 sinks total for infant use which meets the requirement. Director, Karina Cueva will inform LPA when potty chairs have arrived. Once complete and a waiver is completed for the playground, a capacity increase will be granted. An exit interview was conducted and a copy of this report was provided to Director Cueva. Appeal Rights were also discussed and provided to Director Cueva. An LIC 9213- Notice of Site Visit was also issued and must remain posted near the main entrance for 30 days. Non-compliance with posting will result in a $100 fine. This report must be accessible to the public for three years.
