Incident investigation·April 30, 2025

On 04/30/25, Licensing Program Analysts (LPA) Kelli Waters arrived unannounced to follow up on a Unusual Incident Report (UIR) submitted to Community Care Licensing (CCL), regarding an incident that took place on 04/09/25. During this visit, Director Nialea Ortega-Guachena was not present. LPA met with Regional Operations Coordinator, Jennifer Ruiz-Quintero, and informed her of the reason for the visit. LPA conducted a facility inspection, took census of the facility, reviewed records, gathered documents, and conducted an interview. Further information will be needed. Upon completion of the review, the outcome and/or recommendations will be provided. Exit interview conducted and report was reviewed with the Regional Operations Coordinator, Jennifer Ruiz-Quintero. Appeal rights were provided and a Notice of Site Visit was given and must be posted for 30 days.
