What the inspector wrote

On 08/06/25, Licensing Program Analyst (LPA), Kelli Waters, conducted a subsequent unannounced Case Management visit to follow up on Unusual Incident Report (UIR) that was submitted to Licensing by the facility on 07/24/25, regarding an incident that took place on 07/22/25. LPAs met with Center Director, Nialea Ortega-Guachena to discuss incident. The Director reported the following: on 07/22/25, at approximately 7:30am, Staff 1 (S1) observed infant Child 1 (C1) become unresponsive for approximately 3-5 seconds. S1 held C1 until C1 became alert again and immediately notified parents. During the investigation, LPA Waters interviewed staff, reviewed records, and gathered documents. S1 stated that C1 became alert again and resumed playing until parents came. Parents of C1 took C1 to seek medical treatment and C1 returned to school the following day. The facility is working with parents to monitor C1 and have made accommodations based on C1\342\200\231s medical needs. Based on the information obtained during the visit, LPA Waters determined that the facility took the necessary steps to ensure the safety of C1 and other children in care. Based on the information obtained during the visit, there appears to be no violations of Title 22 Regulations pertaining to the reported incident. An exit interview was conducted, and a copy of this report was provided. A copy of this report must be made available to the public, at the facility site, for 3 years.
