What the inspector wrote

On 01/21/26, Licensing Program Analyst (LPA), Kelli Waters, conducted an unannounced Case Management visit to follow up on an Unusual Incident Report (UIR) that was submitted to Licensing by the Learning Jungle (CCC) on 12/05/25, regarding an incident that took place on 12/01/25. LPA met with Center Director, Nialea Ortega-Guachena, and informed her of the reason for the visit. LPA conducted a facility inspection, took census of the facility, reviewed records, gathered documents, and conducted interviews. The facility reported the following: on December 1, 2025, during a transition time from outdoor to indoor, children were washing hands, Child #1 (C1) and Child #2 (C2) went to a mud sink located in the play kitchen area, removed a plastic cover and stuck their hands into the sink area. C1 sustained a deep cut from the exposed metal edge, requiring medical attention. Further information and records will be needed. Upon completion of the review, the outcome and/or recommendations will be provided. Exit interview conducted and report was reviewed with the Director, Nialea Ortega-Guachena . Appeal rights were provided and a Notice of Site Visit was given and must be posted for 30 days.
