What the inspector wrote

On 05/28/2025 at 10:00 am, Licensing Program Analyst (LPA), Griselda Castellon conducted an announced Pre-Licensing inspection for a new single license. Upon arrival, LPAs met with Laura Humble, licensee representative . Applicant is requesting add the toddler component to their facility. The applicant is requesting to be licensed for 12 toddler option children ages (18 months - 36 months) in toddler room (5), and 60 preschool children (2 years-Kinder) in preschool rooms, 3,4,6, 7 and 8. The fellowship hall will be used for preschool children. Hours of operation will be Monday through Friday 7:00 am to 5:00 pm. The facility number 376600052 will be closed and converted to a single license 376301177 upon completing all facility and file review corrections. The facility will no longer serve school-age children. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Laura Humble that staff are required to always maintain direct visual supervision of the children during indoor and outdoor activities. When medications are on site, Laura stated that they will be in a higher cabinet located in the directors office. A fully equipped first aid kit is in the director office . There is an operational carbon monoxide detector on site lo cated in each room. All required licensing documents were observed posted in the directors office. Children will be signed in and out using a Brightwheel App and there is a sing in and sign out binder in the directors office. LPA continued the facility tour and measured all indoor and outdoor activity spaces for toddlers and preschool children. The total indoor activity space for toddlers 476.50 sq ft, and the total indoor activity space for preschool children is 5,929.08 sq ft. These spaces are sufficient to accommodate the requested capacity for both the toddler and preschool components. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping mats for preschool children and for toddlers, bookshelves, and other activity supplies for the children. Toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water is available in the classrooms via brita pitchers . LPA observed all hazardous items to be inaccessible to children. There is an unfenced creek that runs along the front of the playground. The facility has a waiver on file that states, "You have asked for a waiver to allow children to be escorted across the bridge and unfenced creek to the fenced playground with a ratio of one teacher to every nine children. The waiver is valid for the term of the lease and is subject to review at the discretion of the licensing agency". Fire clearance was granted on 08/27/2024. The changing table has at least 1\342\200\235 padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3\342\200\235 high and the changing table is within arm\342\200\231s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Needs and Services Plans and Sleep Plans are kept in each classroom. There is at least one potty chair or toilet for every five toddlers. LPA observed a total of 1 sink and 1 toilet and 1 changing table available for toddler children\342\200\231s use. Additionally, LPA observed a total of 5 sinks and 4 toilets available for preschool children\342\200\231s use These are sufficient to accommodate the requested capacity of children. There is a separate male and female staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the directors office. Facility will provide AM and PM snacks . The facility has access to the kitchen area. The kitchen area is shared with the church. The facility has labeled the usable areas in the kitchen. Teachers and staff will prepare snacks in the kitchen and then transporting them to their designated classrooms. The kitchen area is inside the fellowship hall and is inaccessible by a key locked door. Each classroom includes a microwave, mini fridge, toaster, skillet, and sink for meal prepping only. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in the classrooms are stored in high to reach cabinets and under are key locked. The facility currently has a fully fenced toddler and preschool playground area. The preschool playground Fencing chain link is at least four feet high. The total square footage for the toddler outdoor activity space is 1,134.00 sq ft, which is sufficient to accommodate the requested capacity. Shade is provided via trees. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There are no climbing structures on the playground for children. Drinking water is available via personalized bottled waters . LPA observed all hazardous items on the playground to be inaccessible to children. T he preschool playground Fencing chain link is at least four feet high. The total square footage for the preschool outdoor activity space is 4,810.00 sq ft, which is sufficient to accommodate the requested capacity. Shade is provided via trees surrounding the entire playground and a wood canopy. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure on the playground for children ages (2-5) which is properly anchored. There is adequate cushioning in fall zones of climber provided by wood chips. There are swings for children to use. Additionally, the preschool playground has a sandbox, Lawson Garden, a chicken coop and an outdoor classroom barn/shed. There is a mud kitchen for children to use which has two sinks and is under a wooden canopy. The outdoor sinks have proper drainage. Drinking water is available via personalized water bottles. LPA observed all hazardous items on the playground to be inaccessible to children. Laura Humble was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Criminal Record Clearance - Child Care Centers: Laura Humble was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.\342\200\235 APPLICANT WILL WAIT UNTIL FACILITY OPENS TO DETERMINE IMS NEEDS: \342\200\234Incidental Medical Services (IMS) policy was discussed. LPA also informed Laura Humble of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.\342\200\235 MyChildCarePlan.org--Child Care Centers and Family Child Care Home: Laura Humble was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. \342\200\234 Notice of Site Visit- Child Care Centers and Family Child Care Homes: \342\200\234A notice of site visit was given to Laura Humble and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. \342\200\234 Subscribe to CCLD important information - Child Care Centers and Family Child Care Homes: \342\200\234Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Proof of replacing trash can lid all trash cans shall have a tight fitting cover that is kept on. Room 5 Toddler 1. Proof of replacing trash can lid all trash cans shall have a tight fitting cover that is kept on. 2. Proof of adding a top lid or mesh covering to fish tank. Room 6 Preschool 1. Proof of adding latch on mini fridge. 2. Proof of replacing trash can lid all trash cans shall have a tight fitting cover that is kept on. Room 7 Preschool 2. Proof of replacing trash can lid all trash cans shall have a tight fitting cover that is kept on. Preschool Yard 1. Proof of additional mesh on the chicken coop fencing. 2. Replace/Repair chain-link fence that is bent that is near the swing structure. 2. Proof of adding more wood chips on fall zones of the climbing structure. 3. Proof of filling in the hole that is along the chain link fence. Additional documents listed on the NOIA will be submitted to LPA within 30 days. Laura understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. A license for 12 toddler option children ages (18 months - 36 months) in toddler room (5), and 60 preschool children (2 years - Kinder) in preschool rooms, 3,4,6, 7 and 8. will be granted upon a final file review.
