Pre-licensing visit·May 21, 2026

Licensing Program Analyst (LPA) Tricia Danielson conducted an announced pre-licensing plan of correction (POC) inspection. During a visit to the home on 05/20/2026 LPA identified several items which required correction in order to meet licensure requirements. During today's visit, LPA verified the completion of the following items: 1. Door handle locks have been installed on the laundry room door, the primary bedroom door, and bedroom #2 door 2. Child gates have been installed to prevent access to the kitchen 3. Proof of control of property was received 4. LIC 610 - Emergency Disaster Plan was received The facility now meets licensure requirements and the change of location process has been completed. An exit interview was conducted and a copy of this report was provided to Licensee/Applicant Claxton.
