What Happened
Unannounced case management visit identifying multiple regulatory violations. Armed security guards present without required waiver and construction modifications made without licensing notification.
Observations
- Two armed guards on premises without approved waiver under either license (Type B deficiency)
- Construction completed August 2023 adding wall and door between rooms Green #122 and Yellow #121 without Licensing Department notification (Type B deficiency)
- Construction work done while school was closed, no prior or post-notification to licensing

Director stated that they added a wall and door between classrooms #122 and #121 (Multipurpose room) back in August 2023, but Licensing was not notified prior to the construction nor once completed, which is a potential health, safety and personal rights risk to persons in care.
