On 01/03/2024 at 10:42 AM LPA Dana Stevens conducted an unannounced Case Management inspection for the purpose of following up on an unusual incident that occurred on 07/06/2023 and was reported by Director on 07/07/2023. LPA Stevens met with Director, Kristen Cobb. 7 staff were present with 79 children at the time of this inspection. . On 07/07/2023 Director reported that on 07/06/2023 at around 12:22 pm, 10 children and one teacher, Staff 2 (S2) were present in the 3 year olds class. Child 1 (C1) was crawling/playing near a table during nap time with Child 2 (C2). C1 began crying and stating their arm hurt. Director was not present on the day of the incident but was notified of the incident immediately by phone, by Staff 1 (S1). Parents of C1 were also immediately notified of the injury by S1. C1 was picked up by parents at 1:27 PM and taken to the hospital where C1 was diagnosed with a pulled left elbow. The director stated she interviewed all staff witnesses, informed her District Manager of the incident and requested review of video footage. After review of video footage it was determined that C1's injury was due to improper handling by Staff 2 (S2) and S2' employment was subsequently terminated. During today's visit, LPA interviewed Director, 2 staff and one daycare child, and reviewed staff records. Based on information obtained in interviews and record review, although staff reported and documented the incident in a timely manner to Licensing and C1's parents and corrective action was taken in the termination of S2, LPA's review of staff records revealed S2 had not received training on safe and proper handling of children since 06/2021. California Code of Regulations, (Title 22, Division 12 & Chapter 3), are being cited on the attached LIC 809-D. Upon Receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. Exit interview conducted and copy of this report left with Director. Notice of site visit must be posted for 30 days.
