What the inspector wrote

On 03/06/2025, LPA Dana Stevens conducted an unannounced Case Management inspection for the purpose of a Room Change, LPA met with Director Kristen Cobb. There were 77 preschool children present supervised by a total of 8 staff members. Licensee has requested to add Room 2 to the Preschool areas and remove Room 7 with no change to capacity. Room 5 was slightly expanded to continue to accommodate the existing preschool capacity. LPA measured Room 5 to provide updated measurements. No other changes were made to existing preschool classrooms. The following are measurements of the facility's preschool indoor space: Room #5 (Updated)- 1,388.70 sq. ft. Room #6 (Previously measured) 875.17 sq. ft. Room #4 (Previously measured) 905.08 sq. ft. Room #2 (Previously measured) 746.86 sq ft. The total room measurements for the above preschool classrooms measured at 3,915.81 sq. ft., which is sufficient for the requested capacity of 110. Fire Clearance was granted for 110 children (ages 2-5) on 12/24/2024. Room Change is approved effective today's date, March 06, 2025. No deficiencies cited during today's visit. Exit interview conducted and copy of this report provided to Director. Notice of site visit must be posted for 30 days.

    101229(a) Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs.This requirement was not met as evidenced by: Based on LPA interviews conducted, the licensee did not comply with the above regulation, as incidents reviewed indicated multiple bite injuries with children as a result of a lack of supervision by staff, which poses an immediate Health & Safety risk to children in care.
